  • Complete registration form with signatures

  • Fees agreement forms with signatures

  • 2 passport-size photos of child

  • Copy of child’s birth certificate or passport

  • Copy of Qatari ID cards or passport of parents.       

  • A copy of child’s vaccination record.

  • Copy of child’s HMC medical card. (if applicable)

  • Any court orders involving custody of child. (only for adoption, guardianship, or divorce)
